Children's Targeted Case ManagementThe primary goal of Children's Targeted Case Management is to help children and their families locate, access, and coordinate services that enable children to remain in their communities and achieve their potential. The Case Manager will employ aggressive outreach with the child and family, along with careful attention to continued engagement and on going meaningful involvement of the child and family in the entire process. Our Targeted Case Management Program facilitates individual access to an array of mental health services, provides for smooth transitions between services and ensures continuity of care in addition to the provision of St. Michael’s Center
