Children's Case Management

The primary goal of Children's Case Management is to help children and their families locate, access, and coordinate services that enable children to remain in their communities and achieve their potential. The Case Manager will employ aggressive outreach with the child and family, along with careful attention to continued engagement and on going meaningful involvement of the child and family in the entire process.

Our Case Management Program facilitates individual access to an array of mental health services, provides for smooth transitions between services and ensures continuity of care in addition to the provision of advocacy and active support for a child and family. The goal is to increase the focus on child and family strengths, to increase the family’s role as full partners in service planning and quality improvement, and to increase utilization of natural community supports and other community resources.

Catholic Charities Maine St. Michael’s Center, Wings for Children & Families, Inc. and the Department of Health & Human Services work collaboratively to provide Case Management services throughout Aroostook, Penobscot, Piscataquis, Kennebec, Somerset, Hancock, and Washington counties. The referral process starts with the Intake Team, located at Wings, for intake and screening with a weekly collaborative meeting to determine eligibility.

Wraparound Maine Initiative

The Wraparound process empowers families to utilize their strengths, natural resources and professional supports to achieve goals they establish for themselves. The process is designed to serve families who are parenting children with significant mental health needs, who are at risk of being placed outside their home due to the level of needs.

Qualifying individuals should be between the ages of 5-18 (or 21, in some cases) and have significant mental health diagnoses that require involvement with more than one system (schools, courts, mental health services, ect). The Wraparound process is family-based, and respects that families are the best experts on their family systems.

The Wraparound process encourages families to collaborate with community and natural resources to assist them in meeting their goals. The family’s voice and choice is the most important one in a Wraparound process, and no decisions are made without the family present. Most family decisions are supported by a Wraparound team that the family selects for the purpose of generating unique ideas and approaches to effective interventions.

Catholic Charities staff is fully trained in Hi-Fidelity Wraparound, and collects information to ensure positive outcomes like improved school attendance, a decrease in family disruption and improvement in day to day quality of life for children and their families.
